Ten HG fatalities, of which two were an instructor and student. Nine PG fatalities, of which one was a tandem instructor who failed to connect his leg straps, and three were mini-wing pilots. One additional USHPA member PG fatality happened overseas so is not counted in the US accident total.

I don't have to worry much about failure to hook-in because my harness gets set up with my glider and pre-flighted as such.

Otherwise I guess (for me) it's about not letting myself get interrupted or hurried when doing those final checks (bina, lines, leg loops, helmet strap, chute handle, and buckles)..
